This Privacy Policy describes how MOVE.MENT, P.C.: Chiropractic and Integrative Wellness (“MOVE.MENT,” “we,” “us,” or “our”) collects, uses, and protects information gathered through our website, www.movementig.com (the “Site”). It applies to visitors browsing the Site and to individuals who submit information through it, such as a contact form, appointment request, or newsletter sign-up.

This policy does not describe how we handle Protected Health Information (PHI) created or received in the course of providing chiropractic care, functional medicine care, massage therapy, or any other treatment provided by Move.ment. That information is governed by our Notice of Privacy Practices, provided separately to patients as required by HIPAA and available by request. If you are logging into our patient portal, please refer to that notice.

Information You Provide to Us

We collect information you voluntarily submit through the Site, including:

• Contact and intake forms: name, phone number, email address, and any health-related information you choose to include (for example, a description of symptoms when requesting an appointment).
• Appointment requests and our online booking / patient portal: name, contact information, insurance information, and appointment preferences, in order to schedule and confirm your visit.
• Newsletter and email sign-ups: name, phone number, and email address.
• Any other information you choose to send us, such as through email.

Note: Health information you submit through an intake form or the patient portal (for example, a description of symptoms) may itself qualify as PHI once received by our practice, and is handled consistent with our HIPAA obligations and Notice of Privacy Practices, in addition to this policy.

Information Collected Automatically

When you visit the Site, certain information is collected automatically, including:

• Device and usage data: IP address, browser type, operating system, pages visited, time spent on pages, and referring/exit pages.
• Analytics data: we use Google Analytics (or a similar analytics service) to understand how visitors use the Site, using cookies and similar technologies described below.

Depending on where you live, you may have the right to:

• Request correction of inaccurate information;
• Request deletion of your information, subject to our legal and record-keeping obligations (including obligations to retain certain health records under state law);

• California residents: additional rights under the California Consumer Privacy Act (CCPA), including the right to know what personal information is collected and to opt out of the “sale” or “sharing” of personal information. We do not sell personal information.

Children’s Privacy

The Site is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the Site. If you believe a child has provided us with personal information, please contact us so we can delete it.
